Medical Concerns and Conditions

Please review this section and complete the appropriate documents needed in order to ensure that all health concerns are dealt with accordingly. If you have any questions please feel free to contact our health office.
Allergy Action Plan
If your child is prescribed an epinephrine auto-injector for allergy, please have your doctor complete this packet. Your doctor may also complete the portion of this form for self-administration of both the epinephrine and an antihistamine if he/she feels your child is able to self-administer. This form must also be signed by a parent.
Asthma Action Plan
If your child has a history of asthma, uses an inhaler please have your physician complete the asthma action plan. Your doctor should also complete the self-administration portion of this form if he/she feels your child is able to self-administer the medication.
Seizure Action Plan
If your child has a history of epilepsy or seizures, uses rescue medications for seizures, please have your physician complete the seizure action plan.
Diabetes Action Plan
If your child has diabetes, please contact us to receive the diabetes packet. After reviewing it, we ask that you reach out to your healthcare provider to obtain the Diabetes Orders for the school.
Other Conditions
If your child has a diagnosis other than the ones that are above, please reach out so that the proper paperwork could be given to you.
Note to Parents/Guardians
Please be aware that each packet includes a Permission to Disclose Information form. Since your child has medical conditions that may affect them in the classroom, this form allows me to share relevant medical information with other school staff. This is to ensure the health and safety of your child.
Over-the-Counter Medication
If your child needs to take over-the-counter medication during the school year, please have your doctor complete the form below.
All over-the-counter medication administration requires this form to be completed by your child’s doctor and signed by a parent.
Medication must be brought to the nurse by a parent and should be in the original container.
Early morning or late-day medication should be taken at home.
Routine Prescription Medication
If your child needs to take prescription medication daily or occasionally during the school year, please have your doctor complete the form below.
All prescription medication administration requires this form to be completed by your child’s doctor and signed by a parent.
Medication must be brought to the nurse by a parent and should be in the original pharmacy container.
Students are only permitted to carry medication for life-threatening conditions, such as diabetes, asthma, or severe allergies. If your child falls into one of these categories, please refer to that specific section.
Overnight Field Trips
Please note that a nurse does not attend all field trips, as certain criteria must be met. If your child takes daily routine medication, be sure to include this information on the permission slips sent home so we can plan accordingly and ensure a nurse is present. Keep in mind that the only medications students are allowed to self-administer are for life-threatening conditions such as asthma, allergies, and diabetes. All other medications must be discussed with the health office to make proper arrangements. If over-the-counter medication is needed during a field trip, please complete the OTC form included in the packet. If you already have an OTC form on file for the academic year, there's no need to submit another one.
